NITROUS OXIDE SEDATION CAUSES POST-HYPERVENTILATION APNOEAf

چکیده
We have studied, in six normal subjects, the effect of nitrous oxide sedation on the ventilatory pattern and oxygen saturation using pulse oximetry (Sp0J after hyperventilation to an endtidal carbon dioxide partial pressure (PE'COJ °f 3 kPa. This value of P?Co2 was shown to be less than the apnoeic threshold of all these subjects when their ventilation vs P f COi response curves were plotted. All subjects became apnoeic when told to relax following hyperventilation while breathing 75% nitrous oxide for 90s. Apnoea was defined as cessation of breathing for 20 s or more. The mean duration of apnoea was 78 s (range 29-130 s). All subjects demonstrated arterial desaturation (mean SpOl 75%, range 44-87%). In contrast, following hyperventilation with air, no apnoea was seen in any subject, although there was some evidence of desaturation (mean Sp0] 92.5%, range 88-98%). It was concluded that subjects who are sedated with nitrous oxide behave similarly to those who are anaesthetized rather than to those who were fully conscious, in that they become apnoeic below the apnoeic threshold point. The reduction in SpOl after hyperventilation was explained almost entirely by apnoea and may explain abnormalities of respiratory control and hypoxaemia in patients recovering from general anaesthesia or sedation accompanied by hypocapnia. This mechanism may be of importance in obstetric patients after breathing Entonox, when apnoea and hypoxaemia may reduce oxygen delivery to the fetus.

Analgesic and physiological effects in conscious sedation with different nitrous oxide concentrations
OBJECTIVES to study the physiological changes, as well as the psychosedative and analgesic effects of nitrous oxide, in experimental conditions. STUDY DESIGN 101 dental students volunteers participated in a single nitrous oxide sedation session without dental treatment. Signs and symptoms were registered during and after the procedure. Pulse rate and hemoglobin oxygen saturation were monitore...
متن کاملHypoxaemia after inhalation of 50% nitrous oxide and oxygen.
We have investigated, in volunteers, the effect of inhaling 50% nitrous oxide in oxygen on subsequent oxygenation. Subjects breathed either 50% nitrous oxide in oxygen or 50% nitrogen in oxygen during hyperventilation and normal ventilation. Arterial oxygen saturation and end-expired oxygen concentrations were lower after 50% nitrous oxide in oxygen than after the other mixture with both patter...
متن کاملClinical effects of nitrous oxide conscious sedation in children.
PURPOSE This study examined the clinical effects of nitrous oxide conscious sedation on children. METHODS Fifty-nine healthy children (ages 4 to 13, mean age=7.7 yrs.) requiring dental restorative treatment with nitrous oxide sedation were studied. The behavior and clinical effects were assessed before and 5 minutes after 50% nitrous oxide sedation. In addition, the children were instructed t...
متن کاملAnalgesia for joint punctures in children - experiences with a nitrous oxide / oxygen mixture (LIVOPAN®)
Background Joint punctures in children require adequate analgesia, sedation or both. Suitable for this are intravenous sedatives (eg propofol, midazolam) and analgetics (eg, ketamine and fentanyl). The use of these substances is safe, but requires an intravenous line, monitoring and followup. Nitrous oxide/oxygen (N2O/O2) sedation has a long-standing history of safety and success. N2O/O2 has an...
متن کاملCase-series of nurse-administered nitrous oxide for urinary catheterization in children.
BACKGROUND Children undergoing urologic imaging studies requiring urethral catheterization experience considerable discomfort and psychological distress. Nitrous oxide sedation may mitigate these detriments but the requirement for physician administration has limited the applicability of this technique. METHODS Registered nurses underwent the nitrous oxide training requirements prescribed for...
